This Player Participation Agreement is entered into by and between CeleNation League, and the undersigned individual. 1. Player agrees to participate in the CeleNation League try-out, subject to the terms of this Agreement and the League's rules and regulations, which may be amended from time to time. 2. Player understands that participation in organized sports involves risk of physical injury, including but not limited to sprains, fractures, head injuries, or even death. By signing this Agreement, Player voluntarily assumes all risks associated with participation in the League. Player hereby waives, releases, and discharges the League, and its owners from any and all claims, causes of action, or liability for personal injury, illness, property damage, or death arising from participation in the League, whether caused by negligence or otherwise, to the fullest extent permitted by law. 3. Player agrees to conduct themselves with professionalism and respect for others at all times. Player shall be financially responsible for any damage they cause to the facility, equipment, or property, and for any injury or harm caused to another person, including players, staff, fans, or other attendees. The League reserves the right to suspend or terminate a Player's participation for misconduct, violations of rules, or behavior deemed dangerous or inappropriate, without refund. 4. Player authorizes the League to secure medical treatment in the event of injury and agrees to be solely responsible for all costs related to such treatment. The League is not obligated to provide medical insurance. 5. This Agreement shall be governed by and construed in accordance with the laws of the State of Florida. Any disputes shall be resolved exclusively in the courts of Orange County, Florida. IN WITNESS WHEREOF, the parties have executed this Agreement as of the date signed.
