Waiver agreement - I acknowledge that playing basketball in the Binghamton Premier Basketball League presents certain inherent risks and hazards, which I have fully considered and I fully assume. By registering I acknowledge to the best of my knowledge, there are no pre-existing physical or other health-related conditions, which will interfere with my participation unless noted above. I, the participant, understand and acknowledge that such recreational activities have inherent risks, dangers and hazards, foreseeable and unforeseeable, that may result in injury, illness, or property damage, and on behalf of myself, my family, agents and contractors, I hereby release and agree to hold harmless Jim Evans, Davis College, OLGC and all staff involved with the Binghamton Premier Basketball League, its sponsors, referees, staff, club officers and directors, from all claims, actions, or losses related thereto. The Binghamton Premier Basketball League assumes no liability for injury or damage arising from the results of participation of the above players. By signing this form, I assure BPBL officers that I will have my own insurance coverage to cover any potential damages or injuries obtained while participating in this league, both as player and spectator.
Referee Fees - By registering I acknowledge that my payment today of $125 is for league fees and does not include referee costs. Referee fees are $5 per player per game, paid for in cash (ONLY) and signed off on prior to tipoff each game.
Payment agreement - I acknowledge that once submitted, my payment to the Binghamton Premier Basketball League is completely non-refundable for any reason, including injury.
Referral - If you are new to the BPBL (never played as either a permanent player or sub), you may list one BPBL returning player as a referral. Please fill out the registration form online and list your referral in question 4. You must list your referral before submitting this form in order for it to be paid out.
