Voluntary Participation: I voluntarily participate in the Border City Elite Flag Football League, understanding the inherent risks involved in flag football, including but not limited to, physical injury, collisions, falls, and other potential hazards. 2. Assumption of Risk: I fully assume all risks associated with participation in this league, whether known or unknown, including risks arising from negligence of the league, its organizers, volunteers, referees, or other participants. 3. Medical Fitness: I affirm that I am physically fit and have no known medical conditions or disabilities that would prevent me from safely participating in the league. I understand that I am responsible for consulting a physician before participating if necessary. 4. Waiver of Liability: I, on behalf of myself, my heirs, executors, and assigns, hereby release, discharge, and hold harmless Border City Elite Flag, its directors, organizers, officials, volunteers, facility providers, and any affiliated parties from any and all claims, demands, damages, or causes of action, including those caused by negligence, arising from my participation in the league. 5. Indemnification: I agree to indemnify and hold harmless the league and its affiliates from any claims, costs, or expenses arising out of my conduct or participation. 6. Medical Treatment Authorization: I consent to receiving medical treatment deemed necessary in the event of an injury, and I understand that I am financially responsible for any medical expenses incurred. 7. Code of Conduct: I agree to abide by all rules, regulations, and policies set forth by the league, including sportsmanship, safety, and conduct expectations. Failure to adhere may result in suspension or expulsion without refund. 8. Photography/Media Release: I grant permission for Border City Elite Flag to use photographs, videos, or recordings of me for promotional purposes without compensation. 9. Insurance: I acknowledge that the league does not provide personal accident or health insurance and that I am responsible for my own coverage. 10. Description: Welcome to the official team registration for Border City Flag’s upcoming season! This form is for team captains to register their squad and secure a spot in the league. What is this for? This form is used to register a team in Border City Elite Flag, Windsor’s premier 18+ flag football league. Each team can have a maximum of 9 players on their roster. Captains will be responsible for submitting this form and ensuring all team members complete their individual waivers before the season begins. Cost & Payment Details • $2,025 per team (includes up to 9 players) • Payment must be completed in full to confirm registration • E-transfer payment to: bordercityflag@gmail.com • Include your team name in the e-transfer message Payments can be made in three equal instalments of $675, with the total amount due no later than April 31st to lock in your team’s spot for the season.
